HNUS Off Site Analysis Instructions
During your first morning trip to the bathroom, put urine on a Q-tip and place in a Ziploc bag. Before you brush your teeth in the morning, swab your mouth with a Q-tip and place in a Ziploc bag. Next, cut a lock of hair from around the neck area and place in a Ziploc bag. Last, cut some finger nails / toe nails or both and place in a Ziploc bag. Please make sure you have your name and birthdate on each bag.
If you are a new client, download the HSNU Intake form. Print out and complete all areas of form and send with your samples.
There will be a minimum 7 day turn around on this analysis. Please make sure you submit your complete Name, Address, Phone Number, Birthdate, and Gender with samples. Please mail kit in an envelope to the following address:

The samples are good for testing up to 7 days, so it is not necessary to overnight the kit . The cost for HNUS Analysis is $100, please include a check or money order with your samples.
